SUMMARY:

Purpose: To assess the impact of treatment-related outcomes on the overall health-related quality of life (HRQoL) of patients with head and neck cancer who underwent different combinations of surgery, chemotherapy, and radiation therapy. Methods: This is a prospective, multi-institutional study on quality of life in patients with head and neck cancer treated in Brazil, based on the application of the EORTC QLQ-C30 and EORTC H&N43 questionnaires. To determine the main factors that impact global quality of life, an exploratory factor analysis (EFA) is being carried out. The impact on the global HRQoL of the factors extracted from the analysis is being estimated by the explained variance. Evaluation of patient characteristics (gender, age, alcohol consumption and smoking), disease (primary site and staging) and treatment (surgery - yes or no; chemotherapy - yes or no; and radiotherapy technique - none, 2D, 3D or IMRT) on global HRQoL and on the factors that most impact global HRQoL is being carried out in two stages: (i) using the Item Response Theory (IRT), scores are generated for global HRQoL and for factors that explain at least 70% of the global HRQoL variance, according to EFA; (ii) multivariate regressions are performed with these scores as dependent variables. The effect of patient, disease and treatment characteristics on these scores is being evaluated with multivariate regressions. Partial Results: Our sample contains 917 patients from 18 institutions, distributed across all Brazilian regions. It has 734 men (80%) and 183 women (20%) with a mean age of 60 years (range: 18 to 98 years). We observed that 38% of the tumors are located in the oral cavity, 26% in the oropharynx, 23% in the larynx, 7% in the hypopharynx, 4% in the nasopharynx and 2% of the patients have an unknown primary tumor site. The early stages were less frequent, with 8% of patients diagnosed in stage I, 9% in stage II, 19% in stage III, and 64% in stage IV. The mean follow-up period was 9 months. Problems related to daily activities and speech had the greatest impact on HRQoL. Global HRQoL scores were significantly negatively affected by the primary location of the tumor in the oral cavity in multivariate regression analyses. This is also true for stages III and IV of the disease, for the age above 50 years and for tobacco consumption. The final model shows that the global HRQoL score gradually improves after completion of radiotherapy. When compared to 3DRT, 2DRT negatively impacts overall HRQoL, while IMRT has a significant positive effect. Surgery, chemotherapy, sex and alcohol consumption were not significant and therefore excluded from the regression model.
