SUMMARY:

Chagas disease (CD) is a silent neglected tropical disease caused by the protozoan Trypanosoma cruzi. This parasite is mainly transmitted by triatomine bugs, but there are other transmission routes that play an important role in nonendemic countries. The chronic phase of the disease presents indeterminate, cardiac, gastrointestinal, and mixed clinical forms. CD treatment in the chronic phase is a challenge, especially in the case of elderly individuals, in view of the decline that occurs with advancing age, and that they may have other chronic conditions. The specific aspects of the aging process in chagasic patients need to be better understood. This study aims to verify the presence of anti-T. cruzi antibodies in an elderly Brazilian population, as well as whether there is an association between CD clinical form and other chronic diseases, and if this depends on gender and age. A retrospective study was performed with 945 serum samples from a Brazilian elderly population. Serum samples were obtained from biorepositories, approved by the Research Ethics Committee of the Faculty of Medicine of the University of Brasilia. Samples were inactivated at 56 oC for 30 min and stored in aliquots at -80 oC until use for the Biolisa recombinant Chagas assay (Bioclin), which was performed following the manufacturer's instructions. Clinical and epidemiological data were obtained from electronic medical records. One hundred and seventy-two seniors (172/945; 18.20%) had positive serology in this study. Elderly over 73 years old represented the positive population, of which 67.4% were female. Systemic arterial hypertension was present in 77.8% of the patients and diabetes mellitus in 8.7% of the patients. There were significant differences in blood glucose concentration, HOMA-IR, serum IL-6 concentration and percentage of elderly with type 2 diabetes mellitus with positive serology compared with elderly with negative serology for CD. The results show a proportion of elderly patients with CD positive serology in the composition of a cohort of Brazilian chagasic patients with a prevalence of systemic arterial hypertension and a low prevalence of other comorbidities. This study is important to better understand CD in the elderly and the health care needed for this population.
