SUMMARY:

Introduction: Leprosy is a chronic infectious disease caused by the acid-fast bacillus, Mycobacterium leprae and Mycobacterium lepromatosis. Among the countries that report cases of leprosy in the world, Brazil ranks second in absolute number of cases, behind only from India. The diagnosis of the disease is still essentially clinical. Therefore, diagnosis based only on clinical observation can lead to errors, since leprosy can present signs and symptoms that are very similar to other dermatoses. Given this limitation, the use of rapid laboratory tests that can be carried out in addition to the clinical examination, at the point of care, or even tests based on molecular biology techniques, represent an important tool for the correct and early diagnosis of leprosy, avoiding or minimizing the emergence of disabilities and physical deformities, often observed when the diagnosis is made late. Objective: To test the accuracy of serology and polymerase chain reaction in real time, in series, in diagnosing leprosy in contacts of index cases. Method: The study will be carried out using a prospective longitudinal observational strategy, with monitoring of groups of patients classified as intra-household leprosy contact. Serology will be performed with peripheral blood collection and polymerase chain reaction in real time, from every patient included in the study. The sample will be collected for the polymerase chain reaction and will follow the recommendations for sputum smear analysis in leprosy, using material from intradermal scrapings. Patients classified as intra-household contact of leprosy, treated at the Brasília University Hospital between January 2022 and December 2024, will be included in the study. Patients will subsequently be followed up with an annual clinical examination to assess the development of leprosy. Expected results: This study will produce information on the incorporation of these technologies and paves the way for a faster and more accurate diagnosis of leprosy, as well as contributing to the improvement of contact surveillance actions for the effective control of the disease in Brazil.
