Introduction: The main purpose of clinical services provided by pharmacists is to promote the safe, effective, and rational use of medicines to improve patients' health and quality of life. Despite its expansion, there is no validated instrument with outcome indicators that translate clinical, economic, and humanistic values and benchmarks capable of evaluating and qualifying clinical services provided by pharmacists. Objective: To propose outcome indicators that promote evaluating clinical services provided by pharmacists developed in the hospital environment. Method: This is a mixed study, in which, in the initial phase, a review study was carried out to understand the indicators used to evaluate services. In the next phase, an international Delphi investigation, healthcare professionals familiar with the implementation of clinical pharmacy services, service management, and representatives of patient associations came together to determine a minimum set of outcome indicators. Results: The systematic review found 225 indicators used to evaluate clinical services provided by pharmacists in a hospital environment. Despite this, only 13 (5.7%) indicators were related to results. Based on the findings, 49 experts from all continents reached a consensus on outcome indicators consisting of three clinical, one economic, and one humanistic measures. Conclusion: The set of established indicators represents a proposal for evaluating clinical services provided by pharmacists from a results perspective.
