SUMMARY:

Introduction: Therapeutic patient education (TPE) aims to help people acquire or maintain the skills necessary for selfmanagement, adapting their needs to the disease or chronic condition in the best possible way. To this end, the joint action of a multidisciplinary team, including the pharmacist, is essential, especially in the management of Chronic NonCommunicable Diseases, such as Diabetes Mellitus (DM). It should be noted that adults with diabetes often have polypharmacy, are elderly and can move between different locations and levels of care, requiring the complementation of new modes of care, such as the clinical activities of pharmacists in Primary Health Care (PHC). Objective: To evaluate the impact of a therapeutic education program in a pharmaceutical care service for adults with DM using insulin in PHC. Methods: The research project will involve people with DM who use any type of insulin and who are monitored by the Family Health Strategy Team of a Basic Health Unit. To this end, sequential mixed methods divided into six stages will be adopted: scoping review; interviews and questionnaires with people with diabetes and other actors involved in the Brazilian scenario - situational diagnosis of the scenario; participant observation of the therapeutic education model in Portugal with possible interviews with people with diabetes treated in the Portuguese service; construction of an action framework (TPE Program in the Pharmaceutical Care Process); implementation of the service; perception of people with diabetes and professionals about the implemented Program. Expected results: The results may contribute to consolidating pharmaceutical care and incorporating the practice of therapeutic education for the self-management of a disease of such importance in the context of the health system, developing an effective therapeutic alliance between health professionals, the person and family members in the care process, allowing for a more collaborative approach to treatment.
