SUMMARY:

“Introduction: Cutaneous manifestations have been reported in patients with COVID-19. Studies suggest that such manifestations may arise due to the direct action of the SARSCoV-2 virus on epithelial cell receptors, from inflammatory reactions, or immune responses. Identifying these manifestations may constitute an important criterion in the clinical assessment of patients with a confirmed diagnosis of COVID-19, as well as aid in the diagnosis itself. Objective: To review the literature on cutaneous manifestations observed in patients with a positive laboratory test for COVID-19 and to investigate possible factors associated with these manifestations. Method: This involves two systematic reviews, one targeting the population of children and adolescents and the other adults and the elderly, reported according to the criteria of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA). A single search was performed, used in both reviews, to retrieve studies from the following databases: CINAHL, EMBASE, LILACS, LIVIVO, PubMed, Scopus, and Web of Science Core Collection. Additionally, a search was conducted in the gray literature, using Google Scholar and ProQuest Dissertations & Theses Global. A manual search was also performed in the reference list of the included articles. We used the tool applicable to studies that report prevalence data, from JBI, in assessing the risk of bias of the included studies. A prevalence meta-analysis was conducted using MetaXL® 5.3 software. An association meta-analysis was conducted using the Cochrane Collaboration’s Review Manager® 5.4 software (RevMan 5.4). In the review about aduts and elderly people the certainty of evidence was assessed using the Grading of Recommendation, Assessment, Development, and Evaluation (GRADE) tool for association outcomes. Results: Review 1: Thirty-eight studies were included in the systematic review of children and adolescents, with 35 showing a low risk of bias. The prevalence of cutaneous manifestations was 27% (95%CI: 20-35; I²: 99%), with maculopapular rash on the extremities and trunk being the most common manifestation. Among patients with Multisystem Inflammatory Syndrome in Children (MIS-C), 49% presented with a rash, with urticarial being the most frequent. There were insufficient data for association analysis.
