SUMMARY:

Physical exercise has been considered important in the approach to greater trochanteric pain syndrome. The objective of this study will be to systematically analyze the specialized literature to determine which type, dose, frequency and duration of a physical exercise program should be used for the rehabilitation of greater trochanter pain syndrome; to describe and analyze the relationship between the strength deficit of the hip muscles of individuals with this clinical condition and to investigate the effects of a physical exercise program with different training modes and educational guidelines in improving pain, functionality and quality of life life of individuals this pathology. The first study will be a systematic review, registered under the number CRD42020216803 The databases searched will be the Cochrane Central Register of Controlled Trials (CENTRAL, the Cochrane Library), Pubmed, Embase, CINAHL, evidence database (PEDro) Physiotherapy. The DECS descriptors used will be gluteal region, hip injuries, tendinopathy, pain, exercise and MESH terms: gluteal region, hip injury, tendinopathy, pain, exercise. Randomized and quasi-randomized clinical trials performed with individuals diagnosed clinically and by imaging tests with greater trochanter pain syndrome will be included. Cohort studies, case controls, systematic reviews and clinical trials with hip joint pathologies, pain from spinal disorders or any other hip arthropathy were excluded. The risk of bias and the methodological quality of the articles will be assessed using the PEDro scale and the Rob 2.0. The evaluation of hip muscle strength will be performed through maximum isometric contraction, using the analysis of hip movements (flexion, extension, adduction and abduction, internal rotation, external rotation) and measured with the manual dynamometer e-lastic 02863-17 -10410, portable, bluetooth (Technology BR 10201400007232-2). The prospective, parallel, two-arm, randomized, blind study will be developed jointly with the Hospital Ortopédico e Medicina Especializada – HOME. Adults diagnosed with greater trochanteric pain syndrome through clinical examination and magnetic resonance imaging and with persistent pain in the lateral region of the hip above 3 on the pain analogue scale (VAS) for more than 3 months will be admitted to the study. Patients diagnosed with joint pathologies such as coxarthrosis, avascular osteonecrosis of the femoral head, pain from spinal disorders or any arthropathy detected in radiographic examination will be excluded from the study. Educational guidelines and a program with different training modes for hip and lower limb muscles for twelve weeks will be conducted. The evaluated outcomes will be pain (VAS scale, pressure algometer), hip functionality (VISA-G questionnaire), quality of life (EuroQol) and isometric strength of the hip muscles.
